A settlement has been reached on a lawsuit against the Proctor & Gamble Company (P&G) about its Crest Sensitivity Treatment & Protection toothpaste(Crest STP), providing a monetary refund to those who purchased this kind of product for up to one tube. All those American persons who made a purchase of Crest STP from February 2011 through March 31, 2013 are eligible to submit a Claim Form and related documents to receive a refund of $4 via pre-paid debit card.

As to how to file your Claim Form, you have two choices to take: file it online or generate a personalized Claim Form to print out and send by mail. No matter which choice you prefer, you have to take the following steps:

  2. Click “Claim Form” at the left side of the site to enter into the page for claim filing. You can choose an option as you like from “Securely complete and file claim online” and “Print personalized Claim Form to file via mail”.   • Final Approval Hearing: Scheduled for September 12, 2013.
